Section 107.02, During Construction Form

*FOR USE WHEN UTIL. RELOCATION IS TO BE DONE DURING CONSTRUCTION

107.02 Protection and restoration of Property and Landscape. Amend as follows:

Delete the fifth paragraph and substitute the following:

The contractor is responsible for the coordination, facilitation, and scheduling of all utility conflicts. This includes verifying location of temporary or permanent relocation or any other utility related actions on the project.

The Government has notified the utility companies affected by necessary relocations or adjustments to their facilities, and has provided construction plans and cross sections for their use in identifying utility conflicts. It is the responsibility of the contractor to assure identification and protection of any and all utility facilities on the project.

The following utility contacts shall be consulted prior to performing work that may affect their facilities:

TO BE USED W/OVERHEAD LINES: Relocation of the existing overhead power/telephone poles/lines will be accomplished by (Power/Telephone Company Name) during the road construction. The Contractor shall negotiate a Utility Relocation Plan with (Power/Telephone Company Name) detailing the schedule and responsibilities for the relocation of (Power/Telephone Company Name) facilities and any other utility related actions for the life of the project. The utility relocation schedule and activities will be provided to the CO and included in the preliminary and construction schedules for the project.

TO BE USED W/UNDERGROUND LINES: Relocation of the underground power/phone lines will be accomplished by (Power/Telephone Company Name) during the grading portion of excavation, prior to completion of the subgrade. The Contractor shall negotiate a Utility Relocation Plan with (Power/Telephone Company Name) detailing the schedule and responsibilities for the relocation of (Power/Telephone Company Name) facilities and any other utility related actions for the life of the project. The utility relocation schedule and activities will be provided to the CO and included in the preliminary and construction schedules for the project.

Items to be addressed in the relocation plan are:
  1. Number of days AAdvance Notice@ the utility companies will require to begin utility relocation.
  2. Schedule of areas the utility company will be working in, and duration of relocation activities in that area.
  3. Impacts the utility relocation may have on the contractor=s operations.
    1. Disturbance of compacted and/or groomed areas (clarify responsibilities).
    2. Potential conflicts of utility relocation and construction activities working in the same area (i.e. traffic plan, safety issues).
Add the following to the sixth paragraph:
Contact (in OR, WA & MT - Utility Notification Center, in ID - Dig Line Inc., in WY - Wyoming One Call) not less than three working days prior to the start of any excavation on this project. The utility notification phone number for the ________________ area is (phone number) and statewide 1-800-(OR-332-2344, WA, ID & MT-424-5555, WY-348-1030).
